If you're a new client, please complete the forms you were sent through email and bring them to your first therapy session. Each adult attending a session must complete an Adult Information form. If a child is to attend therapy, a parent and child should complete one Child/Adolescent Information Form per child. Only one Informed Consent is needed for each case.

Please arrive to your first therapy session 15 minutes early so your therapist intern can review the information on your forms and you can spend the therapy time more focused. If you need to fill out the forms at the clinic, please arrive 30 minutes before your scheduled time so you have the full session for therapy.

We are conveniently located in Morrisville, midway between Raleigh and Durham.
On I-40: From Raleigh: Travel north on I-40, take exit 284 and turn left (away from the airport), then go about 1 mile. From Durham: Travel south on I-40, take exit 284A and go about 1 mile.
Turn left on Trans Air Drive (just past stoplight for Perimeter Park), then take an immediate right into the parking lot for Airpark Business Center. Drive along the front of the building; we are near the middle.
On Chapel Hill Road (Highway 54): turn on Airport Boulevard and Reach will be on your right in about 1 mile. Turn right on Trans Air Drive, then take an immediate right into the parking lot for Airpark Business Center. Drive along the front of the building; we are near the middle.
